This is a simple online form I adapted from a well-known Cognitive-Behavioral Therapy technique that helps people to reduce their number of unwanted thoughts. If you think about dying too often, or about that time you threw up in the lunch room, you should probably do this form at least once. Do it as often as you need to, really.
